The American Pharmacists Association and the National Association of Chain Drug Stores Foundation have released an update
to the medication therapy management model the organizations first released in 2005. The revised document, increases the emphasis
on the patient's role in MTM self-management and the importance of transitions of care and collaboration between members of
the healthcare team. Version 2.0 contains five core elements: medication therapy review, personal medication record, medication-related
action plan, intervention and/or referral, and documentation and follow-up. "These Core Elements represent a common thread
for services across practice settings that embrace collaboration between healthcare professionals and promote continuity of
care for the patient," said John A. Gans, Pharm.D., APhA executive VP and CEO.
